You can also request a referral from your social worker or insurance provider. We currently accept Medi-Cal, Kaiser, and Anthem Blue Cross insurance. We also have arrangements with various California Counties and Santa Clara County School Districts. Call us to find out if our services are covered.
Do you need to request a copy of your records?
To request records, we have an “Access to Records Request” form (English | Spanish) that must first be filled out by the legal guardian, if the client, is a minor. In addition, upon completion of this form, it must be sent to RCS Records Department, Rebekah Children’s Services, 290 IOOF Avenue, Gilroy, CA 95020, by fax at (408) 846-2495, or bring it to the front desk at any RCS location. RCS will respond to the request within 15 days of receipt.
Do you need to submit a complaint?
If you believe your privacy rights have been violated, you may file a complaint with RCS by calling us at (408) 846-2100 or one of our other local numbers. Press 0 to speak with our operator and ask for the HIPAA Privacy Officer.
For any other complaints or grievances, you may file a complaint with RCS by calling us at (408) 846-2100 or one of our other local numbers. Press 0 to speak with our operator and ask to submit a formal complaint. You will be forwarded to an appropriate representative who can receive your complaint.
